Accommodation for persons who require nursing or personal care, treatment of disease, disorder or injury and diagnostic and screening procedures. The Beech unit is situated on the ground floor of Risedale at Abbey Meadow (Willow, and Holly unit being above on the first floor) and sharing a reception and entrance with St Luke’s.

The Beech Unit within Risedale at Abbey Meadow offers dedicated quality care for 22 clients who require nursing services and assistance to support their behaviour.

 

The Beech Unit houses 22 large, single en-suite rooms each totalling 18m2.  The en-suite has an assisted shower with shower chair for clients to use independently or with support.  Each room is decorated in bright, modern colours and furnished in a vibrant manner and can be individualised to suit the clients’ preferences.

 

Service users have the choice of several communal areas, including two divided lounges, quiet room, and direct access to an enclosed sensory garden.

 

The large lounges are divided into two separate areas to allow service users small, quieter areas to spend time.  The lounge areas have a variety of chairs and settees to meet individual needs and several dining areas so clients can choose to sit with others or alone.

 

The corridor from the Beech Unit leads directly to an enclosed garden where clients can spend time away from the unit.  A sensory theme is also in place for clients to use as part of their therapeutic care.

We are committed to providing professional care and work alongside the service user and other professionals to meet their desired outcomes.

 

The Beech Unit is overseen by an experienced Mental Health Nurse Manager, and is split into Coniston and Bowness units, each having 11 beds.  Each unit is led by an experienced senior nurse who are both Registered Mental Health Nurses. Within the Beech Unit there are two trained and competent members of staff on duty at all times, in addition to Health Care Assistants

 

Risedale use the Creative Intervention in Response to Untoward Situations, (C.I.T.R.U.S.) model which is accredited to B.I.L.D. as a least restrictive, last resort to needs led behaviours. Risedale has two associate trainers in order to train staff to C.I.T.R.U.S. Level 1.  This is externally supervised Calderstones NHS Trust.  Some of the clients have one-to-one staff allocations and permanent staff are recruited to these roles to ensure continuity of care and boundary parameters for the management of needs led behaviours.

 

The Unit is supported by the Complex Care Manager for Mental Health.

 

The unit does not directly accept clients who are removed from the community under a Section of the Mental Health Act 1983.

 

The unit is primarily used as a long term placement facility for use when clients have undergone a period of assessment and treatment within a secure facility and who are now judged ready for transfer to long term accommodation.  The unit will specialise in supporting clients who require management of behaviour that is not compatible with mainstream long term care provision.  Admission will only be approved following comprehensive assessment processes performed by a person competent and trained to undertake complex assessment procedures. 

 

Each assessment will contain a review of current clients within the unit to ensure the compatibility of the potential client for the environment.  Risedale will not admit any client deemed to be incompatible with existing clients or when their needs cannot be met by the facilities and resources within the home. 

The home will support a vacancy rather than admit an inappropriate client.  Should issues arise with a placement following admission then Risedale will undertake to source a more suitable placement at the first available opportunity in liaison with the Multi-Disciplinary Team.

Risedale at Abbey Meadow Willow and Holly units are owned and operated by Risedale Estates Limited, registered office Risedale at Abbey Meadow, Duchy Court, Barrow in Furness Cumbria LA13 0BF.  Company registration number 01974198, as a private company.

 

The Holly and Willow units within Abbey Meadow offer nursing care services for 25 clients, both younger and older adults who have multi complex nursing care needs. Both units are managed by an experienced and highly qualified Registered Manager  

 

All facilities comply with the Disability Discrimination Act 1995 and the large rooms ensure adequate space for wheelchair users.

 

The skilled nursing team is complemented by the support of our in house specialists who are able to support and advise on Health and Safety, Complex Care and Clinical intervention, all of which have received additional assessment and monitoring training from the CCG and Acute Hospitals Trust.

 

Risedale implement the Six Steps to Success Programme and can therefore nurse clients through complex conditions and support them to a peaceful death when that time comes.

 

Holly and Willow have two moving and handling supervisors and always have access to an in-house continence nurse advisor, nutritional advisor and tissue viability advisor.

 

All registered nurses have direct access to a funded degree programme with the University of Cumbria to further develop advanced skills and professional knowledge.

 

Clients can also individualise their rooms with furniture and belongings.

Alongside individual bedrooms, service users have a choice of communal areas including a large lounge area, quiet room and spacious garden area.

 

Doorways and corridors are wide enough for service users who are nursed in bed to be able to spend time in communal areas if they so wish.

 

Both Holly and Willow are furnished with a variety of seating and dining facilities, including a beverage bay.

 

Service users also have the option of using the quiet room for privacy and access to a large garden where there are seating areas and recreational facilities.

 

We are committed to providing professional care and work alongside the service user to meet their desired outcomes.

 

Service users and their families on Willow and Holly Units will be fully involved in their care planning and will have every opportunity to join in staff meetings and service user forums.

 

Holly Unit

 

The Holly unit is situated on the upper floor of abbey Meadow where residents, families and staff can enjoy superb views of the town from bedrooms and floor to ceiling lounge windows.

 

Holly caters for 14 residents who have been assessed as having complex nursing care needs. Holly’s age range is mixed and the nursing needs of our clients is diverse. Due to complexity, Holly’s staffing ratio is higher than in other homes, and all staff are trained to meet the needs of clients accordingly.

 

All rooms are en-suite and have their own shower facility and overhead tracking hoist. There is a full time activity co-ordinator who is based on Holly and Willow unit who endeavours to work on both group and individual levels in order to provide a person centred approach to activity planning. Holly’s visiting hours are open, and families and friends are welcome. There is provision for family members to stay overnight if necessary in our “Peace Room”

 

The staff team are led by an experienced Senior Registered Nurse and there is a qualified Nurse/Assistant Practitioner on duty 24 hours each day. All staff attend full induction training and there is a mandatory 2 weeks training course for all new Health Care Assistants, as well as ongoing training throughout the year on a wide variety of subjects, e.g. safeguarding and infection prevention and control.

Within Holly unit is a community respite bed where people can book short stays through Adult Social Care Services.

 

Holly unit maintains strong links with a plethora of external professional agencies, and strives to continue to provide excellent nurse led care.

 

Willow Unit

 

Situated on the same upper floor as Holly, willow also enjoys wonderful views of the town from bedrooms and through floor to ceiling lounge windows. Willow is a dedicated unit that provides highly skilled nursing care for 11 residents who are of mixed age and have multi complex conditions that require specialist nursing care. Willow is one of very few facilities in the area that are fully trained and equipped in this level of complex care, and is able to cater for a wide variety of needs including:

 

Acquired Brain Injury

Tracheostomy Care

Minimally conscious

Highly Dependent Nursing Care

 

Willow team is led by an experienced senior Registered Nurse, and the skill mix and staffing level is higher than in other homes due to the complex needs of residents. All staff complete an initial induction programme and a mandatory 2 week training programme is in place for all new Health Care assistants. Training is ongoing throughout the year for all staff, and this is tailored to the needs of staff in relation to the client group.  

 

All rooms are en-suite with shower facility and overhead tracking hoist. Families and friends of residents are encouraged to come and go as they wish, and visiting hours are open.

 

Robust and systematic assessments are undertaken prior to admission to ensure all needs can be met, and excellent links with a wide variety of external professionals is maintained in order to provide the best quality person centred care.

The service type is: Care Home with Nursing.

The regulated activities are:

Accommodation for persons who require nursing or personal care, treatment of disease, disorder or injury and diagnostic and screening procedures.  St Luke’s is part of Risedale at Abbey Meadow Nursing Home and shares an entrance and reception with Abbey Meadow. 

St Luke’s is a new, purpose built nursing home and has 46 beds. 

The home is managed by an experienced and highly qualified manager whose previous role within the Company encompassed case management of complex clients.

St Luke’s primarily cares for client groups over the age of 65 years, however, younger clients may be cared for where their primary health need is physical disability with dementia or dementia alone and it is not deemed detrimental to either potential new or existing clients.  It is not envisaged that significantly younger clients will be cared for within the home.

St Luke’s has four units: Balmoral Unit, Stirling Unit, Piel Unit, and Windsor Unit.

 

Balmoral Unit 12 Beds

 

Within Risedale’s existing nursing provision, we have identified clients with complex nursing needs which co-exist with a dementia.  These clients may be in receipt of continuing health care or simply have more complex nursing and mental health needs.

 

Caring for clients with more complex needs can challenge our general and Dementia nursing homes and therefore this unit offers care in a smaller purpose built unit.  Clients who currently live within our other homes and have complex needs may be offered a transfer to St Luke’s.

Each room within the unit is a large, single en-suite room with direct access from a patio door to private gardens.

The en-suite has an assisted shower with shower chair access for clients to use independently, or with support.  Clients are encouraged to personalise their rooms as they wish.

The unit has a spacious lounge subdivided to provide quieter areas to spend time, should they so wish.  The lounge area has a variety of chairs to meet individual needs and clients can choose to sit with others or alone.

The unit has Registered Nurses and Assistant Practitioners employed. In addition, there is a Senior Nurse who assists the Home Manager to ensure excellence in care and service delivery standards.

 

Additional support is available, on site, from the Risedale Health & Safety Manager, Clinical Nurse Manager, Complex Care Management Team, Training Centre & Director of Nursing. 

 

Risedale purchases advanced nursing equipment and we have staff trained to use Doppler imaging and 24 hours pulse oximetry monitoring.  Therefore all homes are registered for Diagnostic and Screening Procedures.

Facilities and equipment are innovative with use of tracking hoists to assist mobility where necessary and the unit meets the University of Stirling standards of Dementia. Risedale Homes have End of Life Link Nurses and are up to date with all relevant End of Life research and best practice, which enables staff to nurse clients through complex conditions and support them to a peaceful death when that time comes.

 

Each home has at least two moving and handling supervisors and always has access to an in-house continence advisor, nutritional advisor and tissue viability advisor.

 

All registered nurses have direct access to a funded degree programme with the University of Cumbria to further develop advanced skills and professional knowledge.

 

Nurses working within complex environments have regular supervision and in addition have a clinical supervisor to contact for advice and support. 

The units include bedrooms, communal areas and corridors and are decorated to a high standard in bright colours with a mixture of materials and textures and seating.

 

Alongside individual bedrooms, service users have a choice of communal areas including a quiet room, spacious garden areas and hairdressing room.

Service users and their families within the Balmoral unit are fully involved in their care planning and will have every opportunity to join staff meetings and service user reviews.

Stirling Unit 11 Beds

 

The Stirling Unit cares for clients requiring Dementia nursing care.

The Unit is built and equipped to Stirling University Dementia standards. There is a dedicated Senior Mental Health Nurse on the Unit. All staff receive de-escalation and Butterfly training, it is not anticipated that restraint will be used within the Unit.

The Unit will provide step down from our Behavioural Support Unit within Abbey Meadow and step up from Dementia nursing when needs increase.

There is a dedicated lounge and dining room. All rooms have ensuite toilets and showers. Residents will have access to an enclosed sensory garden.

 

Piel Unit 12 Beds

 

The Piel Unit opened as an Dementia nursing facility on 1st May 2017. There is a dedicated Senior Practitioner on the unit.

The Unit is built and equipped to Stirling University Dementia standards. There is a dedicated Senior Mental Health Nurse on the Unit. All staff receive de-escalation and Butterfly training, it is not anticipated that restraint will be used within the Unit.
